Persistent post-COVID breathlessness — what we know in 2026

Years after a COVID-19 infection, a meaningful number of adults still notice breathlessness climbing stairs, a chest tightness with exertion, or a cough that keeps coming back. In 2026 we understand this much better than we did in 2021.
The four common patterns we see
- Persistent small-airway inflammation — picked up on FOT or HRCT
- Post-COVID interstitial change (fibrotic streaks on HRCT)
- Dysfunctional breathing — a learned shallow-breathing pattern
- Deconditioning — the lungs are fine but the body has lost fitness
What an evaluation looks like
We start with a structured history and a 6-minute walk test with oximetry. Most patients then need a PFT with DLCO, an HRCT chest, and sometimes echocardiography to rule out post-COVID pulmonary hypertension.
What actually helps
- Targeted inhaler therapy when small-airway disease is documented
- Structured pulmonary rehabilitation — the single most effective intervention
- Breathing retraining for dysfunctional breathing
- Anti-fibrotics in carefully selected fibrotic cases
If you have been told 'your scan is normal' but you still cannot do what you used to, you have not been fully evaluated. A pulmonology review is worth it.
